Thank you for taking the time to complete the 2021 Vital Village Network-Wide Survey.
The following questions will help us continue to improve the Vital Village Networks experience and resource offerings. This survey should take 15-25 minutes to complete. The collected information is valuable to improve our efforts and plan for the future. Your participation is voluntary and all questions are optional. Responses will be confidential and anonymous. Gift Card Drawing: If you complete the survey before 08/01/2021, you will automatically be entered into a random drawing for a $25 Visa gift card. We are giving out 10 gift cards in total and winners will be announced on August 1st. Your responses will remain anonymous.
